Here are some forms our clients are welcome to fill out and submit online if they are unable to stop by the office and fill it out in person.

Once completed please select the green button to submit the form, or it will not send to us for review.

ALL REQUESTS NEED TO BE SUBMITTED AT LEAST 24 HOURS IN ADVANCED.

FAILURE TO DO SO WILL RESULT IN AN AUTOMATIC DENAIL OF THE REQUEST.

Please make sure you are meeting all weekly program requirements before requesting a pass.

Curfew Extension Request –

A form you may fill out to request a curfew extension.

Overnight Pass Request Form –

A form for members requesting an overnight pass away from the sober living house.

Overnight Pass Extension Request –

This request is available if you are wanting to request an extension of an already processed over night pass request.

Overnight Visit Request Form –

A form to submit when requesting approval for your child to have an overnight visit with you.

Report Form –

This form is for reporting any issues, concerns, or information you believe our staff should address or be aware of.

Once the form is submitted, please communicate with the house manager to confirm that we have received the request.

We require at least 24 hours’ notice to allow time for review and to consult with the care team for feedback.

Once a decision has been made, we will inform the client whether their request has been approved or denied.
